[4] iVEST™ Annual Subscription Fee (ASF)

PAYABLE ANNUALLY FROM SECOND YEAR ONWARDS and non pro-rated. The 1st year ASF payment is already included in the purchase price. ASF payment is inclusive of maintenance, patches, iVEST™ Client software upgrades and one year Class 2 Digital Certificate subscription.

Please fill up the Revocation form which can be found in download page, then email/mail/fax the completed form to iVEST™ Customer Service immediately should  you wish to cease on using the service and to avoid penalty for late payment.

For instance, if you subscribe on 13/10/2003, your next ASF payment due is on 13/10/2004 and 13/10/2005

Payment could be made:

  • At Anjung Internet Jaring and iVEST RA counter: Payment by cash, credit card, cheque, postal order or banker’s draft
  • Via postal: Payment by cheque, postal order and banker’s draft only. Send to iVEST RA Counter

All cheques must be crossed "Account payee only" and made payable to "MIMOS Berhad". On reverse of cheque, postal order or banker's draft, indicate your NRIC number and payment description: "iVEST annual subscription fee"

[5] iVEST™ Card Replacement Process

Ensure that your iVEST™ card is functioning well upon delivery. Defective iVEST™ cards must be reported to iVEST Customer Service within 7 (seven) days from date of purchase to be valid for warranty. Defective iVEST™ cards reported after 7 (seven) days, will cause the registered user to bear the replacement cost. To get a replacement card:

  • You must physically present at any Anjung MIMOS outlets
  • Fill and sign Digicert Revocation form
  • Fill and sign Digicert Application form
  • Fill and sign iVEST Card and Reader Return form
  • Submit payment by Cash, Cheque (payable to MIMOS Berhad), Bank Draft (payable to MIMOS Berhad), Credit card (VISA or Master Card) or Purchase Order (for PO, please fax to 03-89961424 in advance) OR
  • Proof of purchased (e.g receipt or delivery order) for replacement within the warranty period.

iVEST™ Card Replacement via Authorised Personnel (AP)

  • Ensure to handover the filled and signed form and payment to AP for processing
  • Payment by Cheque, Banker's Cheque or Purchase Order (for PO, please fax to 03-89961424 in advance)

[6] iVEST™ Card Reader Replacement Process

Ensure that your smart card reader is functioning well upon delivery. Defective smart card readers must be reported to iVEST Customer Service within 1 (one) year from the date of purchase to be valid for warranty. To get a replacement reader:

  • You must physically present at any Anjung MIMOS outlets
  • Fill and sign iVEST Card and Reader Return Form
  • Submit the faulty reader
  • Submit proof of purchase e.g receipt or delivery order
  • Submit payment (if applicable) by Cash, Cheque (payable to MIMOS Berhad), Bank Draft (payable to MIMOS Berhad), Credit card (VISA or Master Card) or Purchase Order (for PO, please fax to 03-89961424 in advance)

iVEST™ Card Reader Replacement via Authorised Personnel (AP):

  • Ensure to handover the filled and signed form, faulty smart card reader and proof of purchase to AP for processing
  • Payment (if applicable) by Cheque, Banker's Cheque or Purchase Order (for PO, please fax to 03-89961424 in advance)

[7] iVEST™ Card PIN Unblocking Process

CAUSE: As a security feature, your smart card will block itself after three (3) consecutive trials of keying in the wrong PIN.

SOLUTION1: Please bring your iVEST card to any of our counter to unblock. You may send representative if you are unable to present yourself. However, ensure to write an authorised letter to authorise your representative to unblock your iVEST card PIN.

SOLUTION2: Alternatively, you may courier to iVEST RA Counter to :
iVEST RA, MIMOS Berhad,
Technology Park Malaysia,
57000 Kuala Lumpur.
Courier together:

  • Your blocked iVEST card
  • An authorization letter to have your smart card unblocking.

Note:You are advised to change your PIN immediate upon receiving. How to change PIN, double-click iVEST Gate > Change PIN.

[8] About iVEST™ PIN

  • PIN is random numbers generated for each certificate
  • You are advisable to change PIN immediately upon receipt for security reasons
  • PIN consists of 6 to 8 alphanumeric characters only
  • PIN is case sensitive.

[9] MyKad PKI PIN Unblocking

CAUSE: As a security feature, your MyKad PKI PIN will block itself after three (3) consecutive trials of keying in the wrong PIN

SOLUTION: Please bring your MyKad to any Anjung Internet Jaring counter to unblock. You may send representative if you are unable to present yourself. However, ensure to write an authorised letter to authorise your representative to unblock your MyKad PKI PIN

[10] About MyKad PKI PIN

  • PIN is generated during the certificate download
  • You are advisable to change PIN immediately upon receipt for security reasons
  • PIN consists of 6 to 8 alphanumeric characters only
  • PIN is case sensitive.

[11] MyKad PKI Unblocking

CAUSE: As a security feature, your MyKad PKI will block itself after three (3) consecutive trials of downloading the certificate.

SOLUTION: Please bring your MyKad to JPN or Government Service Centre to unblock.

[12] Delivery of Products

Kindly contact iVEST Customer Service if you do not receive your smart card or your iVEST Client Kit within 21 days (3 weeks) from date of purchase. iVEST will not be able to reimburse or be held responsible if report of missing delivery is not made to iVEST Customer Service within 21 days

[13] Delivery of PIN Mailer

You should be expecting to receive you PIN Mailer within 7 days from the date of receiving your iVEST Card. Kindly contact Digicert Customer Service if you do not receive your PIN and request for your PIN to be re-sent

Alternatively (for URGENT cases only)
You may request your PIN online via Phone with Digicert SB. Ensure to fax an authorization letter in advance before calling Digicert to request for your PIN.

If you did not received PIN for more then 1 month, please bring your iVEST card to iVEST/Anjung Internet Jaring Counter to reset/unblock PIN (refer Unblocking process). Digicert is unable to resend your PIN Mailer if it is already more then 1 month.

[14] Authorised Personnel (AP) Programme

For convenience on Batch application, iVEST has introduced an AP Programme whereyby AP is able to verify their staffs/users/customers' application form before sending to iVEST for processing.

How to become an AP:

  • AP must be permanent staff in the organisation
  • Supporting documents:
    • Provide photocopy of both sides of NRIC
    • Letter from the organisation for nomination as an AP (letter of Nomination as an Authorised Personnel for iVEST Registration - see sample in AP Programme document)
    • Specimen Signature - This is to get the hardcopy of the signature of the appointed AP
  • iVEST (RA Manager) will view all the relevant documents and issue an Appointment Letter (approve) or Rejection Letter to the respective organization
  • Physical sighting at RA/Anjung Internet Jaring counter (during submission of supporting documents or collection of Appointment Letter)
  • AP to collect and verify the following documents:
    • Appointment Letter
    • iVEST Application Guidelines
    • Samples of letters
    • DIGICERT Application Form
    • iVESTmail Reservation Form

[15] Roles of an AP

  • Physically verify applicant, application forms and supporting documents
  • Prepare verification letter for applicant
  • Submit completed and verified application form, supporting documents and payment to AIJ or iVEST RA Counter

[16.1] New Registration of iVEST™ Card (Digisign iVEST CA and Digisign iVEST CA Enhanced)

  • You must physically present at any iVEST RA/Anjung Internet Jaring Counters
  • Fill and sign Digicert Class 2 Application form
  • Supporting documents:
    • One (1) Recent passport size colour photograph
    • One (1) photocopy of both sides of NRIC or passport (Malaysian must provide copy of NRIC)
  • Submit payment by Cash, Cheque (payable to MIMOS Berhad), Bank Draft (payable to MIMOS Berhad), Credit card (VISA or Master Card) or Purchase Order (for PO, please fax to 03-89961424 in advance).
New Registration via Authorised Personnel (AP)
  • Ensure to handover the filled and signed form and payment to AP for processing
  • Payment by Cheque, Banker's Cheque or Purchase Order (for PO, please fax to 03-89961424 in advance)

[16.2] Renewal of iVEST™ Card (Digisign iVEST CA and Digisign iVEST CA Enhanced)

Your certificate is valid for a period of time. Please View Certificate to check your certificate validity. If you wish to continue iVEST enabled services, you must renew your expired certificates:

  • You must Physically present at any of iVEST RA/Anjung Internet Jaring Counters
  • Fill and sign Digicert Class 2 Application form
  • Submit payment by Cash, Cheque (payable to MIMOS Berhad), Bank Draft (payable to MIMOS Berhad), Credit card (VISA or Master Card) or Purchase Order (for PO, please fax to 03-89961424 in advance)

Renewal via Authorised Personnel (AP)

  • Ensure to handover the filled and signed form and payment to AP for processing
  • Payment by Cheque, Banker's Cheque or Purchase Order (for PO, please fax to 03-89961424 in advance)

[16.3] iVEST™ Server Certificate Registration

This application can only be accepted at iVEST RA counter, Mimos Berhad, Technology Park Malaysia, KL:

  • Walk-in registration at iVESTRA Counter
  • Fill and sign Class 2 Digisign Server ID. The fields that need to be filled:
    • domain name of the server (if not available, please state machine name unique to the Corporation);
    • name of the organisation that operates the server;
    • name of the department/section/unit within the organisation that operates the server; (optional)
    • Organisation registration number;
    • mailing address of the organisation;
    • Type of web server which the organization is using (eg: Microsoft IIS, stronghold, etc);
    • Platform of the server (eg: Windows2000 Server, etc);
    • Telephone number and facsimile number of the organisation;
    • Email address through which the organisation may be contacted; and
    • The name of the contact person in the organisation who is responsible of the server.
  • At least two sources of supporting documents:
    • One (1) copy of Certificate of Incorporation (ROC) - Form 9 (Sdn Bhd) or Form 8 (Berhad)
    • AND one (1) copy of any one (1) of the following document:
      • Memorandum of Association (MoA)
      • Articles of Association (AoA)
      • Societies Bylaws booklet
      • latest audited financial statement
      • latest income tax return
  • An authorisation letter from the management are required to allow a representative of the organisation to submit this application
  • Certificate Signing Request (CSR) containing the details which you have stated in the application form

Note: Available to all Malaysian and foreign legal entities (except individuals)

[16.4] iVEST™ Biz Card Registration

This application can only be accepted at iVEST RA counter:

  • Walk-in registration at iVESTRA
  • Fill and sign Class 2 Digisign Server ID. The fields that need to be filled:
    • domain name of the server (State your organization name);
    • Media Access Control (MAC) address of the machine that will be used to install the iVEST Biz Client Software;
    • name of the organisation that operates the server;
    • name of the department/section/unit within the organisation that operates the server; (optional)
    • Organisation registration number;
    • mailing address of the organisation;
    • Type of web server which the organization is using (eg: Microsoft IIS, stronghold, etc);
    • Platform of the server (eg: Windows2000 Server, etc);
    • Telephone number and facsimile number of the organisation;
    • Email address through which the organisation may be contacted; and
    • The name of the contact person in the organisation who is responsible of the server
  • At least two sources of supporting documents:
    • One (1) copy of Certificate of Incorporation (ROC)
    • AND one (1) copy of any one (1) of the following document:
      • Memorandum of Association (MoA)
      • Articles of Association (AoA)
      • Societies Bylaws bookle
      • latest audited financial statement
      • latest income tax return
  • An authorisation letter from the management are required to allow a representative of the organisation to submit this application

Tips
To get the MAC address on Windows 98/2000/XP:

  • Go to DOS prompt (Start > run > type "cmd" and click OK)
  • Type "ipconfig/all"and click "Enter"
  • MAC address is stated as Physical Address under Adapter Local Area Connection

Note: Available to all Malaysian and foreign legal entities (except individuals)

[16.5] iVEST™ Server Certificate / Biz Card Renewal

The process is almost the same as new application. Renewal of iVEST Server Certificate requires new Certificate Signing Request (CSR), registration form and authorization letter. Where else renewal of Biz Card only requires registration form and authorization letter. Other supporting document is not compulsory. (refer iVEST Server Certificate Registration and iVEST Biz Card Registration)

[16.6] MyKad Online Certificate Download/Registration

There are 2 options:

  • You can download the certificate yourself following the steps given in the iVEST™4MyKad Online Kit (refer technical FAQ)
  • You can physically present at any of the authorised outlet and have them download it for you.

Counter Registration

  • You must physically present at any RA/Anjung Internet Jaring outlets
  • Fill and sign MyKad Registration form
  • Submit your MyKad, Digital Certificate Voucher and form to counter personnel to perform the certificate download
  • Upon completion, counter personnel will handover your MyKad with MyKad Online Certificate inside your MyKad
  • Ensure to change your PIN as the PIN give by counter personnel is only temporary

[17] Revocation Process (iVEST™ Card and Server Certificate)

To avoid misuse, you are advised to revoke your card if your card is missing or you do not intend to continue using the related services. The same process will apply if you wish to revoke your Server Certificate.

There are two modes to request for revocation of certificate:

  1. Initiated by Subscriber
    • Subscriber complete (fill in details and sign) the Revocation Request Form
    • Submit form and the effected card personally or via AP to any Anjung MIMOS or iVESTRA Counter.
  2. Initiated by Employer to Subscriber, due to termination of service, death, migration, transfer, etc
    • The Employer completed (fill in details only) the Revocation Request Form
    • Prepare Letter of Request for Revocation on behalf of the Subscriber
    • Submit form, Revocation Request Letter and the effected iVEST card personally or via AP to any Anjung MIMOS or iVESTRA Counter
